Antioxidants — The Defence the Body Makes Itself

Sub-article of Supplementation — Wheel of Health. See also: Recovery, Antifragility and Harmonism, The Gap and the Signal, Water, Cancer — The Harmonism Protocol, Monitor.

The most powerful antioxidant in a human body is one no shop sells. Three enzymes, superoxide dismutase, catalase and glutathione peroxidase, dismantle reactive oxygen by the million without being used up, and glutathione, a small molecule every cell builds for itself, keeps them supplied. The body raises this defence when it meets oxidant stress at a dose it can answer: a hard training session, a sauna, a fast, a cold plunge, the bitter chemistry of a broccoli sprout. Swallowing antioxidants in bulk does not raise it, and in the largest trials it has done harm. A pill earns a place in two situations only, against a deficit a test has measured or against a lesion a trial has named. In a body carrying active cancer the logic turns over.

The opposite belief has an author and a date. Denham Harman proposed in 1956 that aging is the slow accumulation of damage done by free radicals, and the corollary, that more antioxidant must protect, reached a mass readership through Linus Pauling’s megadose vitamin C campaign, launched with his 1970 book on the common cold. A marketing number followed. The United States Department of Agriculture published laboratory scores for the antioxidant capacity of foods, the ORAC values printed on juice bottles, and withdrew the database in 2012 because no evidence showed that a food’s test-tube capacity explained what it does in a body. Super Immunity already applies the correction to one mushroom, calling the claim that chaga carries “the highest ORAC of any food” marketing folklore built on a retired database.

1. What the Antioxidant System Is

The cell’s defence is catalytic, made on site, and organised around a molecule that is a messenger as often as it is a threat.

Every mitochondrion leaks a small fraction of the electrons it passes along, and the leak produces superoxide. Superoxide dismutase converts it to hydrogen peroxide; catalase and glutathione peroxidase convert the peroxide to water. A single enzyme molecule repeats the reaction millions of times. Glutathione, assembled from three amino acids with cysteine as the scarce one, donates the electrons glutathione peroxidase spends, and selenium sits in that enzyme’s active site, which is why Supplementation lists selenium as the cofactor of the cell’s own defence. Vitamin C and vitamin E work differently. Each molecule quenches one radical and is spent until something recycles it: vitamin C regenerates vitamin E at the membrane, and glutathione regenerates vitamin C. Dietary antioxidants are passengers in a network the cell runs.

Hydrogen peroxide is the hinge. At low concentration it switches proteins on and off, carries the message of muscular effort to the nucleus, and tells the cell to build more mitochondria. Helmut Sies, the biochemist who gave oxidative stress its modern definition, later named this signalling range oxidative eustress and set it against the distress of overload. The same molecule instructs at one dose and injures at another.

Four lines of work broke Harman’s theory on exactly this point. David Gems engineered roundworms lacking antioxidant enzymes; their free radicals rose and their lifespans did not fall. Arlan Richardson’s group studied eighteen mouse strains with raised or lowered antioxidant enzymes between 2001 and 2009 and found no meaningful difference in how long they lived. Rochelle Buffenstein’s naked mole rats carry less antioxidant defence and more oxidative damage than mice of the same size, and live about eight times longer. Siegfried Hekimi reported in 2010 that worms made to overproduce superoxide lived about 32% longer, and that vitamin C abolished the gain. The oxidant was doing the work, and the antioxidant was undoing it.

Recovery names the general law: a stressor at a sub-damaging dose produces a strengthening response. Antifragility and Harmonism holds it as the Two-Move Alchemy at the body’s register, challenge and then integration. Reactive oxygen is that law at the scale of the cell. Breathing describes oxygen as the terminal that pulls electrons through the respiratory chain; a fire that small leaks sparks, and the sparks are how the hearth learns to hold more heat.

2. How the Defence Is Trained

Because the signal and the defence are coupled, the defence grows when the signal arrives in a dose the body can answer, and the master switch is a protein called Nrf2.

At rest a partner protein, KEAP1, holds Nrf2 in the cytoplasm and marks it for destruction. Oxidants and certain plant compounds alter KEAP1, Nrf2 escapes to the nucleus, and it switches on a battery of genes at once: the enzymes that synthesise glutathione, the detoxifying enzyme NQO1, heme oxygenase-1 and dozens more. The effect outlasts the stimulus by days. A vitamin C tablet mops up a quantity of oxidant and is gone within hours; an Nrf2 signal rebuilds the machinery that does the mopping.

Five inputs pull the switch, and four of them already have homes on the Wheel of Health. Exercise produces the oxidant pulse that drives mitochondrial growth. Heat raises heat shock proteins alongside it. Cold adds a norepinephrine surge and a metabolic load. Fasting, held at Purification § Fasting, adds autophagy, the clearing of damaged mitochondria. The fifth input is food. Sulforaphane, formed when the glucoraphanin in crucifers meets the plant enzyme myrosinase in a chopped or chewed leaf, is the best studied of the plant compounds that release Nrf2. Mark Mattson’s work on what he calls neurohormetic phytochemicals reads most of the celebrated plant “antioxidants” the same way: a plant’s defensive chemistry, bitter because it is mildly toxic, acting in a human body as a small stress that the body answers by strengthening itself.

The Chinese tonic tradition names the same structure in its own terms. Supplementation records what that tradition calls double direction activity in the Superior herbs, regulation toward the body’s own set point from either side of it. A substance that only pushes in one direction is a drug; a substance that helps the organism find its balance is a tonic. Loading an isolated antioxidant pushes one way. Training Nrf2 restores the regulator.

The switch has two edges, and the evidence for pulling it with an extract is thinner than its reputation. A 2023 systematic review found eight randomised trials of sulforaphane in cancer, six at high risk of bias and none testing a clinical endpoint such as survival or progression. Bardoxolone methyl, a potent drug activator of Nrf2, was tested in kidney disease in the BEACON trial; the trial was halted in October 2012 because patients on the drug had more heart failure and more deaths. Of the inputs that raise the defence, exercise is the one with decades of outcome data behind it, and its benefit runs through many pathways at once. The pharmacological version of the same switch has not earned that standing.

3. What Overloads the Defence, and What Silences It

The defence fails in two opposite directions: oxidant load beyond what it can answer, and antioxidant load that stops it hearing the signal.

Overload comes first because removing it does the most. Tobacco smoke delivers oxidants straight to the lung. Alcohol is metabolised to acetaldehyde, which Alcohol lists beside oxidative stress among its injuries. Industrial seed oils oxidise in the fryer and in the body, and Seed Oils describes the lipid peroxides and aldehydes that exhaust superoxide dismutase, catalase and glutathione peroxidase, systems it calls finite. Free iron catalyses the Fenton reaction that turns hydrogen peroxide into the hydroxyl radical, the most destructive species of all, which is why Supplementation refuses supplemental iron without a measured need. Chronically high blood sugar glycates proteins and multiplies oxidant output, and heavy metals, held at Root-Cause-of-Disease, generate reactive oxygen directly. Clearing these precedes everything else, the first move of the alchemy: clear before you cultivate.

Silencing is the error that the supplement market sells. Goran Bjelakovic and colleagues updated their Cochrane review in 2021: 78 randomised trials, 296,707 participants. Across the 56 trials at low risk of bias, antioxidant supplements raised all-cause mortality slightly (risk ratio 1.04). Beta-carotene raised it (1.05), vitamin E raised it (1.03), vitamin A, vitamin C and selenium did not change it. Three large trials had made the harm visible before any pooling did. In the Finnish ATBC trial of 1994, male smokers given beta-carotene developed about 18% more lung cancer. The American CARET trial, testing beta-carotene with vitamin A, was stopped in 1996 after 28% more lung cancers. SELECT reported in 2011 that men taking 400 IU of vitamin E daily had about 17% more prostate cancer. The authors of the Cochrane review found no evidence to support antioxidant supplements for primary or secondary prevention.

Exercise is where the signal matters most. Michael Ristow’s group gave about forty young men 1,000 mg of vitamin C and 400 IU of vitamin E daily through four weeks of training in 2009, and the supplements abolished the gain in insulin sensitivity the training produced in the placebo group. Ristow read it as the antioxidants intercepting the signal. The finding is contested: Yfanti and colleagues gave the same two vitamins through twelve weeks of hard cycling to 21 trained men in 2010 and found no difference in any adaptation they measured. Merry and Ristow still advised in 2016 against antioxidant supplements during training, since the benefit is small and the risk real. Evidence is mixed here, and the practical rule costs nothing, the same arithmetic Recovery applies to cold water straight after lifting: take no high-dose antioxidant pill to support training, and let fruit and vegetables supply what food supplies.

4. What Builds the Defence

Because the defence is trained, building it follows the order of the Wheel: clear the load, apply the stressors, supply the raw materials.

Movement leads. Zone 2 work and strength training, set out in Movement, are the most reliable oxidant pulse a person can apply. Sauna four to seven times a week carries the Kuopio cohort’s frequency signal held in Recovery; cold follows heat and never sits in the hours straight after a lifting session. Time-restricted eating and periodic longer fasts add the clearing signal. Each of these carries its own exclusions in its own article, and the cardiac exclusions for heat and cold, and the exclusions for extended fasting, apply here unchanged.

Raw materials come second. Glutathione needs cysteine, glycine and glutamate, so adequate protein is its first support; the protein targets sit in Essential Supplements. Collagen and gelatin are dense in glycine. Selenium completes glutathione peroxidase, and the total from food, supplements and Brazil nuts stays under the 400 µg tolerable upper intake; Supplementation carries the 150 to 200 µg working range. N-acetylcysteine supplies cysteine directly, and Harmonist protocols already use it at 600 mg daily where heavy-metal or liver support calls for it, as at Liver-Gallbladder Flush, for the length of that protocol rather than open-endedly. It can deepen the blood-pressure drop of nitroglycerin and other nitrates, and it is withdrawn in active cancer, for the reasons § 6 gives.

Food carries the Nrf2 signal at the doses a plate delivers. Crucifers daily, broccoli sprouts most potently, chopped and left a few minutes or chewed well so that myrosinase can work, with a pinch of mustard seed over cooked broccoli to restore the enzyme heat destroys. Alliums, berries, green tea and the bitter herbs work as signals more than as scavengers, whatever their labels claim. Concentrated sulforaphane extract is a different dose, and at high intake in a body short of iodine it can act on the thyroid; anyone taking it daily checks TSH on the annual panel. No concentrated antioxidant extract or therapeutic antioxidant dose has established safety in pregnancy or breastfeeding, and none is taken then without obstetric care.

5. When a Supplement Earns a Place

An antioxidant pill closes a gap or it silences a signal, the same question The Gap and the Signal asks of every pharmaceutical molecule. Two kinds of gap answer it.

A deficit a test has measured. Keshan disease, a heart-muscle failure that killed children in the selenium-poor soils of northeast China, was largely prevented by selenium supplementation from the 1970s onward. Scurvy is cured by vitamin C and by nothing else. Glutathione falls with age in many people, and Kumar, Sekhar and colleagues randomised 24 older adults to glycine with N-acetylcysteine or a placebo for sixteen weeks; published in 2023, the supplement corrected glutathione deficiency, oxidative stress, mitochondrial function, insulin resistance and measures of strength and walking, where the placebo did not. Twenty-four people is a small trial, and it tested a measured deficiency in older adults, not a general tonic.

A lesion a trial has named. In the Age-Related Eye Disease Study, a daily formula of 500 mg vitamin C, 400 IU vitamin E, 15 mg beta-carotene, 80 mg zinc and 2 mg copper cut progression from intermediate to advanced macular degeneration by about 25% in high-risk participants, and had no effect on cataract. Its successor replaced beta-carotene with 10 mg lutein and 2 mg zeaxanthin because beta-carotene raised lung cancer in smokers, and that version is the one to use. Its 80 mg of zinc is twice the 40 mg tolerable upper intake, accepted for a named eye disease with copper alongside and never carried over into a general stack. In the PIVENS trial, 800 IU of vitamin E daily for 96 weeks improved liver histology in 43% of 247 non-diabetic adults with biopsy-proven fatty-liver inflammation, against 19% on placebo. Set against the Cochrane mortality signal, SELECT’s prostate finding and vitamin E’s added bleeding risk with warfarin and other anticoagulants, the PIVENS dose is a time-bounded treatment of a biopsy-proven lesion, held with the physician who follows the liver, and nothing else.

The two rules sort the rest. Essential Supplements sets vitamin C by sex, low for men because of oxalate stones; Supplementation keeps selenium inside its ceiling. An antioxidant pill taken because the label says antioxidant answers neither rule, and the label can hide a drug. Alpha-lipoic acid is sold for diabetes as an antioxidant; it also lowers blood glucose, additively with insulin and the sulfonylureas, and the word says nothing of it.

Molecular hydrogen, held open. Water carries hydrogen-rich water as the third stage of its protocol, and the case rests on Ikuroh Ohsawa’s 2007 report in Nature Medicine that dissolved hydrogen neutralised the hydroxyl radical and peroxynitrite while sparing the signalling oxidants. Such selectivity would spare exactly the signal § 1 describes, if the mechanism held. The chemistry argues against direct scavenging at the doses people drink: hydrogen reacts with the hydroxyl radical at about 4×10⁷ M⁻¹s⁻¹, proteins and lipids at 10⁹ to 10¹⁰, and tissue hydrogen after a glass of water reaches the micromolar range where millimolar would be needed. The likelier action is signalling, through enzymes and Nrf2, which a 2021 preprint argues from the same figures.

The human record is real and small. One vendor of hydrogen generators advertises more than two thousand studies on its science page and lists eighteen. Tyler LeBaron’s 24-week trial of 60 adults with metabolic syndrome is the strongest of them: total cholesterol fell from 187.7 to 169.2 mg/dL, fasting glucose from 121.5 to 103.1, HbA1c from 5.8% to 5.1%, against a flat placebo; the tablet maker co-funded the trial and supplied the product, the lead author consults in the field, and the inflammatory-marker table contradicts the text on CRP. A 2024 meta-analysis of eight trials and 357 patients found a significant fall in triglycerides and none in total cholesterol, LDL or HDL. In Sim and colleagues’ four-week trial of 38 healthy adults, antioxidant capacity rose only in participants over thirty, oxidative DNA damage did not differ between groups, and inflammatory gene expression and immune-cell death fell, an anti-inflammatory signal outrunning the antioxidant one. HYBRID II, inhaled 2% hydrogen for eighteen hours after cardiac arrest, reported good neurological outcome in 56% against 39% (P = 0.15, short of significance) in 73 patients before the pandemic closed enrolment, with survival at ninety days and full freedom from symptoms both significant as secondary results. Hydrogen has shown no harm at the doses people drink, is plausible as a signalling agent, and supported by a small and mostly industry-adjacent human record. It stays in the morning glass as Water prescribes it, and is never ranked above the stressors.

6. Where the Rule Inverts: Active Cancer

A tumour lives under heavier oxidative stress than the tissue around it and leans on the same defence the healthy cell uses, so in active cancer the antioxidant system becomes the tumour’s shield.

Martin Bergö’s group gave N-acetylcysteine or vitamin E to mice with small lung tumours in 2014; the tumours grew larger and more numerous, and the cancer cells lost p53, the protein that stops damaged cells dividing. In 2015 the same group found N-acetylcysteine doubled lymph-node metastases in melanoma, and Sean Morrison’s laboratory reported in Nature that circulating melanoma cells die of oxidative stress on the way to new tissue, and that antioxidants let more of them arrive. ATBC’s excess lung cancers in smokers are usually read the same way in humans: small undiagnosed tumours, given a shield, grew. Lung cancers carrying mutations in KEAP1 run Nrf2 permanently on, and resist chemotherapy and radiation in part through it.

Cancer — The Harmonism Protocol already draws the line for N-acetylcysteine and direct glutathione: otherwise excellent support, withdrawn once active cancer is present, because cancer cells exploit the glutathione system to shield themselves. The mouse evidence reaches vitamin E as directly. Harmonism extends the same reasoning, from mechanism rather than from a trial, to concentrated activators of Nrf2, sulforaphane extract among them, since the pathway they open is the one that raises the shield. Food-level crucifers stay on the plate. Every concentrated antioxidant or Nrf2 activator in a body with active cancer is decided with the treating clinician and read by Monitor, inside the architecture Cancer — The Harmonism Protocol builds, where the oxidative therapies exploit the same asymmetry from the other side.

Prevention and treatment part here. The body that has no cancer is served by a stronger defence. The tumour that exists is served by it too.

7. The Practice

The sequence runs in the order the Wheel turns, and each step has a marker that says whether it is working.

First, clear the oxidant load: no tobacco, little or no alcohol, no industrial seed oils, no supplemental iron without a ferritin result that calls for it, blood sugar held down. Fasting insulin and HbA1c track the last; ferritin with transferrin saturation tracks iron. Second, train the defence: daily movement, regular heat, cold away from lifting, periodic fasting within their exclusions. Third, feed it: enough protein, selenium within its ceiling, crucifers and sprouts at the table. Fourth, test before any antioxidant pill, and take one only against a deficit the test shows or a lesion a trial has named, for as long as the condition holds. Fifth, in active cancer, stop the concentrated antioxidants and decide the rest with the clinician.

One cheap marker follows the whole system. Gamma-glutamyl transferase, GGT on a standard liver panel, recycles glutathione at the cell surface and rises with oxidant and toxic load, alcohol above all. A falling GGT within the normal range is a rough sign that the load is clearing; Blood Tests places it on the panel. The omega-3 index and the vitamin C, selenium and zinc decisions follow the markers set out in Essential Supplements.

The cell keeps a small fire, and Logos orders it at that scale as at every other: a spark that instructs, a defence that grows by answering, a balance the body finds for itself when nothing pushes it one way. The supplement aisle sells protection without the stress. The body refuses the bargain. It grows strong by meeting the oxidant and answering it, and Monitor, Presence turned on the body, is how a person reads whether the defence is answering.
